Regarding registering an account, anyone at least age of 13 years can sign up a new account and get it activated according to Follow My Health Portal policy.
Through this patient portal, users can manage their medical care more efficiently and communicate with their healthcare providers as easy as they want to.
REQUIREMENTS FOR REGISTRATION
To successfully create an account on the Follow My Health Patient Portal, here are the items needed;
- An active Email to receive frequent notification when required.
- Full Names which includes your First and last name.
- Your Zip code is required too prior to your location.
- Date of Birth as available on your Identity Credentials.
- A working Home Phone Number, this appears to be optional.
- Your Social Security Number (SSN), this is optional as well.
HOW TO REGISTER ON FOLLOW MY HEALTH PORTAL
- First, open the official Follow My Health site at https://www.followmyhealth.com
- Click on I Want To Sign Up below the login page.
- Now fill the registration form by providing your Email, Zip code, Full Names, Date of birth, Home Phone Number and SSN.
- Once you entered the correct information on the form, click on Confirm and Continue.
- Now verify your account, you are now a registered owner of an account on Follow My Health Patient Portal

Provided you are above 13 years of age and you already have an active account, you can definitely proceed to login and enjoy all the benefits that comes with it.
Regarding signing in your account, you can use the official Follow My Health Login page and open to other alternatives or Login Using Northwell Health both are recognized by the company.
