Northwell Health does not manage, own or control the Follow My Health Patient Portal, but being a user you are officially allowed to login your account using the Northwell login interface.

DIFFERENCES OF FOLLOW MY HEALTH AND NORTHWELL HEALTH

FollowMyHealth and Northwell Health are two separate applications, each with it’s own unique features to help you manage your medical care. You can access both via web or your smartphone device. See the table below to better understand the differences between the two health platforms.

LOGIN WITH NORTHWELL HEALTH?

You should know FollowMyHealth Patient Portal can also be accessed using your login credentials directly on the login page or through the login page of Northwell Health site.

  • First, Visit the Northwell login page at https://northwellhealth.followmyhealth.com/
  • Then click on Sign In and proceed to enter your Username and Password.
  • Finally click on the below Sign In button again to gain full access to your Patient Portal.

NOTE : You can choose to use an alternative by logging in with your Apple, Cerner, Facebook, Google or Microsoft account. Not to worry, only your login information are being used on these services.

FORGOT USERNAME OR PASSWORD?

If you’re having problems logging in, Click the “Forgot Username?” or “Forgot Password?” link beneath the log-in interface. In order to reset and retrieve your username or password, you will need to undergo two-step verification to confirm your identity.

Email confirmation not received? If you did not receive an email confirmation to finish the account activation process, please give us a call at 1-(844) 364-8108. Our portal is available around-the-clock for you to contact us and fix any problems.

Locked out notification? Try again in ten minutes if you are unable to access your account. This is a safety precaution that keeps your account safe.
