Forgot Username
Recovering a forgotten Username shouldn’t be much of a problem at all, solution for such circumstances have been made possible by the Follow My Health officials.
If you ever forget your username, the support team can help you recover it by asking you a few questions. For example, your phone number lined to account, email address and any other means of identification to prove account ownership.
HOW TO RECOVER YOUR USERNAME?
With the Follow My Health Portal or it mobile app, you can use the Forgot your Username feature to retrieve it. Follow this simple steps;
Step 1: Visit the Follow My Health Login Page: Open your web browser and go to (https://www.followmyhealth.com).
Step 2: Find and click on “Forgot Username” option, it’s available below the login page. However, if you do not remember your password, you may need to use the “Forgot password?” link first.
Step 3: Enter the requested information which includes some personal information to verify your identity. These are your:
- First and Last Name
- Date of Birth
- Email Address or Phone Number
- Social Security Number (SSN).
Step 4: Complete the security CaptCha verification to proof you are a human accessing the portal and requesting for this username recovery.
Step 5: Click on Continue: After entering the required information, click on the “Continue” button.
Step 6: Check Your Email: If your submitted information matches the information on our file, you will receive an email containing your username. Do well to check your Junk or Spam Folders just incase you didn’t receive it to your inbox.
